Several of our members have passed on that they have not received a membership card after becoming a member of USW Local 5000. If you did not receive a membership card or would like to request a replacement card, you can now request a card online by visiting https://uswlocals.org/local-union-5000/request-membership-card. 

This form will be made available under the 'Member Resources' menu link at the top of the page and selecting 'Request a Membership Card' from the drop-down menu in case you need to use it again in the future. 

We understand that many of you may wish to have your card sent to you on the boat, however, due to the way the membership card request is processed and mailed from the International, we can only send the card to your home address. While this is not ideal, it is the only way the system is set up to work at this time, and if it changes to allow us to have the cards sent to your ship address, we will add that capability in the request form.
