The deadline for the 2023 Health Awareness Initiative (HAI) is fast approaching.  In order to qualify for funding in your Health Reimbursement Account (HRA) in 2024, your completed verification form must have a date of service between October 1st, 2022 and September 30, 2023 and your form must be submitted no later than November 15, 2023. 

If your spouse is enrolled on the Cleveland-Cliffs Medical Plan, both you and your spouse must have a wellness examination and submit completed verification forms in order to be eligible for HRA funding. 

It is highly recommended that you submit your verification form via email at [email protected].  You will receive an email confirmation that your form has been received.  Please be sure to keep your email confirmation as your receipt.

To make completing your wellness exam easier, we have partnered with HealthPoint Wellness. Appointments will be offered at the Union Hall on Tuesdays from 12 p.m. to 3 p.m. and Wednesdays from 11:30 a.m. to 3 p.m. for Members and their spouses. Deerfield appointments may be made on Wednesdays from 9 a.m. to 11 a.m. and Thursdays from 9 a.m. to 11 a.m.for Members only. Union Hall appointments will be extended beginning September 19th to 6 p.m.
