This form should be used anytime the union needs records, policies, pay information, or other documents to investigate an issue or enforce the contract. If you are unsure how to use this template or what to request, contact a steward or a member of the Executive Board before sending it.

Grievance Report Form
This form is used to file a formal grievance with the union and document a contract issue through each step of the grievance process. It should be used anytime you believe the Collective Bargaining Agreement has been violated or misapplied. The form allows the union to clearly document what happened, what contract language was violated, and what remedy is being requested, as well as track the grievance as it moves through each step, including arbitration if necessary. If you need help filling out or submitting this form, contact a steward or a member of the Executive Board as soon as possible so your issue can be handled correctly and within the required time limits
