Because individuals under 21 are not permitted on the casino floor at any time, Rivers Casino requires that an Underage Form be completed for all attendees under age 21.

Please return the completed form for children attending to Darlene in the District 10 Office by email at dmazak@usw.org (preferred) or by mail to: USW District 10, Attn: Darlene 1001 Ardmore Blvd., Suite 200, Pittsburgh, PA 15221. Forms must be received no later than Wednesday, May 20, to allow sufficient time for approval.

Approved underage guests will meet in a designated area inside the hotel lobby at specific times for certain events. They will receive a wristband and be escorted to the Event Center by casino security. Please note: underage guests will not be permitted to attend any conference events without prior casino approval. If you arrive with an unapproved underage guest, they will not be allowed into the conference area. Onsite approval cannot be granted.
