2026 HAI Form

The 2026 Health Awareness Initiative (HAI) wellness verification form is for eligible Cleveland-Cliffs employees, retirees, surviving spouses, and covered spouses to confirm completion of their annual wellness examination. To qualify for 2027 HRA funding, each individual must have a wellness exam performed between October 1, 2025 and September 30, 2026, and submit a completed verification form—signed by their healthcare provider—no later than November 15, 2026. A separate form is required for each participant, and all sections must be filled out accurately before submitting by email or mail as instructed on the form.
