Below are a number of forms that our stewards, officers and members sometimes need. If you have any trouble opening or downloading the forms, contact the Local Union office to get a paper copy.
VOUCHER FORMS: Below is a link for the voucher that you need to fill out in order to get paid by the union. Use this when you are part of a negotiating committee, are taken off of work to attend a steward training or other union training or conference. Only fill out the parts of the voucher that are highlighted in yellow. Leave the other parts blank for our Financial Secretary to fill out.

SAFE STAFFING FORMS: Below is a link for the Local Union's safe staffing form. If you feel that you have been made to work with a staffing level that something unsafe happened, or could have happened, we urge you to fill out one of these forms and send them to the Local Union office.
